It's more than Diabetes!

Cardio-Metabolic chronic diseases are plaguing the US population and are creating an untenable level of cost to the US healthcare system. Although there is a growing public awareness of the pervasiveness and impact of Diabetes, there is less concerning the chronic disease states that co-exist with Diabetes. The infographic demonstrates the need for a strong primary care structure in the US and for full visibility on these populations of chronic disease sufferers. We can improve outcomes AND lower costs by using data and technology to drive earlier action and better management of these diseases.
