Each year, the Centers for Medicare & Medicaid Services (CMS) releases its Final Physician Fee Schedule, outlining updated reimbursement policies and rates for the upcoming year. In the 2026 rulemaking, CMS introduced several meaningful updates for virtual care—most notably, new Remote Patient Monitoring (RPM) billing codes and an increased conversion factor, both of which contribute to higher reimbursement opportunities for eligible services.
These changes signal CMS’s continued commitment to supporting technology-enabled care models that improve access, outcomes, and efficiency for patients with chronic conditions.
Maximizing Virtual Care Programs with RPM and CCM
Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) are complementary services that can be billed concurrently, enabling practices to deliver more continuous, proactive care. CCM supports patients with two or more chronic conditions through comprehensive care planning and coordination, while RPM allows clinicians to remotely track physiologic data via connected devices between office visits or following hospital discharge.
Importantly, both RPM and CCM may be delivered under the general supervision of the billing provider and do not require services to be performed solely by in-house clinical staff. This flexibility allows practices to scale programs efficiently, including leveraging outsourced clinical monitoring teams when internal staffing is limited.
Patients enrolled in CCM often benefit significantly from the addition of RPM—particularly those with uncontrolled hypertension, diabetes, or other conditions that require frequent monitoring. As a result, many practices strategically begin with CCM to identify and engage patients most likely to benefit from RPM before investing in connected devices and expanded workflows.
What’s New for 2026
For 2026, CMS has made RPM programs more flexible and accessible than ever by introducing two new RPM codes designed to support care models that involve lower data transmission thresholds and reduced treatment management time. These updates lower barriers to participation and create new opportunities for practices to tailor RPM programs to patient needs while maintaining financial sustainability.
Below is an overview of the average national Medicare reimbursement rates for 2026 for Remote Patient Monitoring (RPM) and Chronic Care Management (CCM). Reimbursement amounts for specific geographic localities can be found using the CMS Physician Fee Schedule search tool.
All amounts listed are per patient per month unless otherwise noted.
