Improving Cardiovascular Disease Outcomes with Remote Patient Monitoring
Risk Stratification, RPM, and CCM for
Hypertension and Heart Failure
With the rapid rise of hypertension and other cardiovascular disease, the cost for treatment in coming years will be staggering. Rimidi's clinical management platform combines patient-generated data from connected devices like scales and blood pressure cuffs with clinical data from the EHR to more effectively manage patients with conditions like heart failure and hypertension.
In addition to bringing in patient-generated data for the highest acuity patients, Rimidi calculates a 10-year ASCVD risk score, supporting risk stratification and population health management. Not only is this data synthesized within the Rimidi platform for a holistic, disease-focused view of the patient's health, but Rimidi presents in-workflow recommendations based on the latest clinical guidelines.
Cardiovascular Conditions Rimidi Supports
Rimidi integrates with a variety of leading connected devices designed to help you better manage patients with hypertension, heart failure, and other cardiovascular disease.
Hypertension
Research has shown that RPM reduces systolic blood pressure and diastolic blood pressure compared to usual care or self-monitoring alone. The Rimidi platform uses a cellular-enabled BP cuff to seamlessly transmit blood pressure data to the Rimidi platform, with no complicated syncing or pairing for the patient.
Heart Failure
RPM offers a way to reduce costly heart failure utilization. Connected scales and pulse oximeters feed weight and oxygen saturation data right into Rimidi’s platform. Clinicians can configure alerts within Rimidi for if a patient shows a concerning amount of weight gain or a drop in oxygen levels, allowing them to focus attention on patients who may be showing signs of worsening heart failure and intervene.
